How to Request Patient Medical Records
To request patient records you will need to submit ALL of the following items:
- Notarized HIPPA release signed by the patient and notarized
- Record Search Fee: $50 per date of service, payable to North Channel EMS
- Patient Name, Address, and any other information to help find records
- One date of service, no ranges
All items should be submitted together either in person or via mail. Incomplete requests will not be considered.
